# How does Medipyxis handle rounding at a skilled nursing facility?

Medipyxis runs wound care rounding at skilled nursing and assisted living facilities from start to finish: set up the facility as a building, book every resident's visit for the day, see each resident, bill the visit with the facility on the claim, and send the facility its signed notes.

The videos have no sound, and each one has a text description below it. Every patient in them is a made-up demo patient.

## How do I set up a nursing facility as a building in Medipyxis?

*Part 1 of 5: The building in CRM · Available now*

The Add New Practice form in CRM takes the practice type Skilled Nursing Facility. A facility set up that way gets three extra tabs: Providers (who rounds there, and on which days), Residents (who lives there now) and Clinical Notes.

**Why it matters:** Who covers the building, who lives there and what has been signed there are all on one page.

**Text description of this video**

- 0:00 In CRM, the Add New Practice form is open. The practice type is set to Skilled Nursing Facility.
- 0:04 The demo facility, Sandbox Pines Skilled Nursing, is found by searching the practice list and opened.
- 0:07 The facility's page shows its address and a Facility prohibits photography checkbox. When it is ticked, notes for residents there state that no photographs were taken because of facility policy.
- 0:09 The Providers tab lists who rounds at the building and on which days. Here one provider is the Primary, rounding Monday, Wednesday, Thursday and Friday. A backup or cover provider can be added for set dates.
- 0:14 The Residents tab lists the patients whose chart says they live there now, with the date of each one's last signed visit at the building.
- 0:17 The Clinical Notes tab lists the signed visits that took place at the building for a date range.

## How do I book every resident at a facility for one day?

*Part 2 of 5: Book the facility day · Available now*

Video (no sound, 20 seconds): [02-book-facility-day.mp4](https://help.medipyxis.com/assets/videos/whats-new/snf-rounding/02-book-facility-day.mp4)

In Fleet Calendar, Book facility day lists the building's residents. Tick the ones to see, and each gets a visit back to back: 9:00, 9:20, 9:40, 10:00. Anyone already booked that day is left alone.

**Why it matters:** A morning of rounds is one booking, not one per resident.

**Text description of this video**

- 0:00 Fleet Calendar opens.
- 0:02 Book facility day opens a form: facility, date, provider, start time and minutes per resident.
- 0:05 The form is filled in: Sandbox Pines Skilled Nursing, October 26, the rounding provider, 9:00 AM, 20 minutes each. The facility's four residents are listed, each with a visit type.
- 0:07 Residents are ticked one by one. Each gets the next time slot: 9:00, 9:20, 9:40 and 10:00.
- 0:10 Book 4 residents is clicked.
- 0:15 All four show Booked with their times: 4 booked, 0 already booked, 0 not booked. Every visit records the facility it happens at.

## How does a provider round and sign visits at a facility?

*Part 3 of 5: Round and sign · Not switched on yet*

Video (no sound, 78 seconds): [03-round-and-sign.mp4](https://help.medipyxis.com/assets/videos/whats-new/snf-rounding/03-round-and-sign.mp4)

My Clinical Day groups the day's residents by building, with a Round button for each. The visit goes one step per screen, pre-filled from the last visit. One click of Pull Codes fills the billing, the provider signs, and the claim is drafted automatically.

**Why it matters:** Many short bedside visits in a row, on a screen sized for a tablet.

**Text description of this video**

- 0:00 My Clinical Day shows the day's route on a map and the residents booked at Sandbox Pines Skilled Nursing, grouped under the building with a Round button.
- 0:06 Round opens the first resident's visit in the rounding layout: one step per screen, five steps in all.
- 0:09 Step 1, Wound and size. The wound is pre-filled from the last visit and marked for review: a left heel diabetic foot ulcer, length 2.1 cm and width 1.6 cm. The area is calculated.
- 0:14 Step 2, Bed and drainage. Exudate amount, type and odor, carried from the last visit to confirm.
- 0:19 Step 3, Edges, skin and signs of infection, each compared with the prior visit.
- 0:21 Step 4, Procedure and treatment. Today's procedure, and the factors that slow healing, such as glucose control and perfusion, marked as addressed.
- 0:26 Step 5, Wrap-up. The care plan, the follow-up interval and the next visit date.
- 0:31 Billing: one click of Pull Codes fills the per-wound billing cards with diagnosis codes. Because the visit is at a skilled nursing facility (place of service 31), the visit code comes from the nursing facility code family. The codes stay on the cards and are marked reviewed.
- 0:49 The provider's attestation is recorded.
- 0:51 A list of documentation gaps appears, each with a button that goes to the right step. Here the provider chooses Continue anyway; items left unanswered print as "Not documented."
- 0:56 All sections of the visit are saved.
- 1:06 The progress note preview: a wound care progress note for a skilled nursing facility visit, place of service 31.
- 1:09 The visit is signed and locked.
- 1:14 The signed note appears in the patient's progress notes, and the claim for the visit is drafted automatically.

## How does the claim show the facility, and how does the facility get its notes?

*Part 4 of 5: The claim and the facility's notes · Available now*

Video (no sound, 26 seconds): [04-claim-and-notes.mp4](https://help.medipyxis.com/assets/videos/whats-new/snf-rounding/04-claim-and-notes.mp4)

The claim carries place of service 31 and names the building in Box 32. On the building's Clinical Notes tab, one download collects its residents' signed notes, without billing, as a zip.

**Why it matters:** The claim shows where the care happened, and the facility gets its notes in one step.

**Text description of this video**

- 0:00 Billing opens.
- 0:02 The claim drafted for a facility visit: the payer, the date of service and place of service 31.
- 0:07 The Service Facility card, Box 32 on the claim form, names the facility where the service was rendered, Sandbox Pines Skilled Nursing, with its address. The rendering and billing provider cards sit beside it.
- 0:12 Back in CRM, the facility's Clinical Notes tab lists the signed notes for October 19 to 22: two residents.
- 0:15 Download notes (.zip) prepares the notes: each clinical note without its billing section, any amendments as separate files, and an index.
- 0:20 The zip is downloaded: 2 notes.

## Can Medipyxis look up a resident in PointClickCare?

*Part 5 of 5: Look up a resident in PointClickCare · Not switched on yet*

Video (no sound, 5 seconds): [05-pcc-lookup.mp4](https://help.medipyxis.com/assets/videos/whats-new/snf-rounding/05-pcc-lookup.mp4)

Look up a resident in PointClickCare, then link them to a patient here or create one. Until PointClickCare is connected, the screen says so plainly.

**Why it matters:** New residents come in from the facility's own record instead of being typed in again.

**Text description of this video**

- 0:00 The PointClickCare page: Data Sync, where resident data can be queued to come in from PointClickCare, and an empty list of sync jobs.
- 0:01 Look up a resident: find one referred resident in PointClickCare, then link them to a patient in Medipyxis or create one.
- 0:01 In this demo system PointClickCare is not connected, so a notice says: "PCC is not connected to this environment yet. Look-ups will not return residents until PointClickCare is connected."

## Questions and answers about nursing facility rounding in Medipyxis

### Does Medipyxis support wound care rounding at skilled nursing facilities?

Yes. A skilled nursing or assisted living facility is set up in Medipyxis as a building, with the providers who round there, its current residents and its signed clinical notes. Book facility day books every resident's visit back to back, and each visit records the facility, so the claim carries place of service 31 and names the facility in Box 32. A rounding layout for the visit itself, one step per screen, is built but not switched on for customers yet.

### Can I book all residents at a facility at once?

Yes. In Fleet Calendar, Book facility day takes the facility, date, provider, start time and minutes per resident, lists the facility's residents, and books each ticked resident back to back. Residents already booked that day are left alone.

### Can a nursing facility get copies of its residents' notes?

Yes. The facility's Clinical Notes tab in CRM downloads the signed notes for a date range as one zip: each clinical note without its billing section, any amendments as separate files, and an index.

### Which visit code does Pull Codes suggest for a nursing facility visit?

For a visit at place of service 31 or 32, Pull Codes suggests the visit code from the nursing facility code family instead of the office code. The suggestion is a starting point: the provider reviews every code before signing.

### Does Medipyxis connect to PointClickCare?

A PointClickCare resident look-up is built: find a referred resident in PointClickCare, then link them to a patient or create one. It is not switched on for customers yet, and it works only once PointClickCare is connected for the practice.
